From Diagnosis to Daily Management: How Our Diabetes Management Program Enhances Lives

At the Community Health Centers of South Central Texas (CHCSCT), we go beyond merely managing diabetes—we’re dedicated to transforming lives through comprehensive, personalized care for each of our patients. With millions affected by this chronic disease, CHCSCT embraces the opportunity to showcase the effectiveness of proactive healthcare. As a leading provider in an area with diverse health needs, we are raising the standard of diabetes care through our Diabetes Management Program. Our focus on personalized care and early intervention empowers our patients to take charge of their health and turns the challenge into opportunities for enhancing overall wellness and quality of life.

What is diabetes?

Diabetes is a long-lasting (chronic) health condition that affects how your body turns food into energy. This imbalance happens when the pancreas can’t produce enough insulin or the body can’t use it effectively, leading to high blood sugar levels. Over time, that can cause serious health problems, such as heart disease, vision loss, and kidney disease.  There are three main types of diabetes: Type 1, Type 2, and gestational diabetes (diabetes while pregnant).

Why early detection of diabetes matters

Early detection of diabetes is crucial to provide individuals with the necessary help they need, but recognizing the subtle signs and symptoms can be challenging to many. According to the Center for Disease Control (CDC), about 38 million US adults have diabetes, and 1 in 5 do not know it. At CHCSCT, we prioritize early detection and personalized care to help you manage your health proactively. We recommend scheduling a comprehensive check-up, complete with blood tests and vital assessments, to evaluate your risk or current status of diabetes. Through regular and thorough check-ups, we strive to effectively manage and prevent diabetes in our patients.

The Diabetes Management Program at CHCSCT

We understand that every individual faces unique challenges when managing diabetes. Our diabetes management program is designed to meet the unique needs of each patient and improve health outcomes. It includes personalized care plans for diabetic and pre-diabetic patients that feature free nutrition counseling with a dietitian, medication discounts, and regular checkups to ensure they remain healthy and well-managed. Here’s an overview of the program’s offerings:

Free nutrition counseling: We offer personalized dietitian services for established patients with an active diagnosis of diabetes that is currently uncontrolled. Through one-on-one virtual meetings with a dietician, we help you better manage your diabetes, monitor your blood sugar, and feel healthier. For those who do not have access to a smart phone or WIFI connection, our health center welcomes patients to utilize our clinic facilities. This option ensures that individuals without the necessary technology or a suitable environment at home can still benefit from our program.

Free retinal eye screenings: We aim to promote early detection and proactive management of eye-related complications for our diabetic patients. CHCSCT offers a free Retinal Eye Screening Program for adult patients to detect early signs of diabetic retinopathy, a common complication of diabetes affecting the eyes.

Cost assistance and accessibility: We believe that financial constraints should not prevent anyone from accessing the diabetes care they need. To make treatment more affordable, we offer free services and discounted medications and visits. We also provide virtual consultations to ensure that all patients can receive expert guidance from the comfort of their homes. This approach allows for flexible scheduling, making it easier for individuals to incorporate the program into their daily lives.

By addressing the diverse aspects of diabetes management, CHCSCT ensures that every patient has the tools and support necessary to manage their condition effectively and continue living a full life.

How the Diabetes Management Program changes lives

Our Diabetes Management Program is designed to create impactful and lasting health improvements, empowering patients to achieve remarkable wellness transformations. This dedicated care completely transformed Pedro’s life.

Pedro was struggling with unexplained fatigue, body aches, and was generally not feeling well. After being diagnosed with unmanaged diabetes, he needed a healthcare provider who could offer both specialized and affordable care, especially as he was uninsured and concerned about the costs of ongoing medical expenses. After visiting a  CHCSCT clinic, he learned that he qualified for discounted visits and prescriptions and began a personalized diabetes care program.

Since joining the program last year, Pedro has made significant changes to his life: he improved his diet, began walking regularly, and started following a prescribed medication regimen. He now feels better, has more energy, and is dedicated to maintaining a healthy lifestyle. Pedro expressed his gratitude for the support he received: “Here (at the Community Health Centers of South Central Texas) they helped me to get the insulin, the pill medication, and everything. They have helped me a lot here.”
